The Main Office, located on the 1st Floor of the Islamic Center of America, is your primary point of contact for all Masjid forms and policies. Our staff is ready to assist you with your requests and provide additional information as needed. You may visit the office to complete forms in person, or download them using the links below. Some forms are processed upon submission at the office, while others must be completed and emailed to the appropriate department.
